A Case of Monocytic Leukemia of Schilling Type

Abstract
A 25 year old Korean male was admitted to Ewha Womans University Hospital with
high fever, nausea, diarrhea and abdominal pain.
The patient had been debilitated rapidly and terminated with acute monocytic leukemia
within 10 days from the onset.
Peripheral blood examination revealed moderate anemia (Hb. 9.4-9.7g/dl), extremely
high leukocytic count (268,000-275,000) with predominating monocytic cell series
(monoblast 14%, promonocyte 85%, neutrophil and eosinophil 1%) and severe
thrombocytopenia (24,000).
Bone marrow aspirates revealed hypercellular marrow with the predominance of
monoblast (58%) and promonocytes (33.5%) and markedly decreased erythroid element
and abscence of megakaryocytes.
The monocytic cells showed the characteristics of the cells of Schilling type.
Enlargement of lymphnodes were slight and the spleen was not palpable.
There was no gum hypertrophy but the Rt. tonsil was enlarged and had several
hemorrhagic spots on the surface.
A cervical lymphnode biopsy revealed poorly differentiated monocyte infiltration
throughout the lymphnode.
